Every blood vessel in the body — from the aorta down to the smallest capillary — is lined on the inside by a single continuous layer of endothelial cells.

What it is

The endothelium is a sheet one cell thick that separates flowing blood from the tissue around it. Laid out flat it would cover several hundred square metres.

It is not a passive pipe lining. Endothelial cells sense the flow passing over them and respond to it.

What it does in the body

The endothelium decides what crosses between blood and tissue. In most of the body the junctions between the cells are selectively leaky, letting nutrients and white blood cells through; in the brain they are sealed far more tightly, which is the basis of the blood–brain barrier.

These cells also control the diameter of the vessel. They release nitric oxide, which relaxes the smooth muscle in the vessel wall and widens it — and other signals that narrow it — so blood is directed to where it is needed.

They keep blood from clotting where it should not, by presenting a non-stick surface, and they can reverse that when a vessel is injured. And when a tissue needs a better blood supply, endothelial cells are the cells that migrate out and build new capillaries.
